Our next survey is designed to help us better understand more about your experience of applying for or receiving CHC funding.

NHS Continuing Healthcare (NHS CHC) is a package of care for adults aged 18 or over which is arranged and funded solely by the NHS. If you’re eligible for NHS Continuing Healthcare, the care services are funded by the NHS at no cost to you.

The information you provide within this survey will help us better understand what is working well and highlight any areas of improvement you might feel are required.
